WELCOME TO OUR OFFICE

Your initial visit to our office will consist of a consultation explaining your diagnosis and treatment options. To help expedite your appointment when you arrive, your paperwork required to be completed by our office can be printed off under the Online Forms section of our website. If you are unable to print the forms, they can be completed by you upon your arrival to our office.

It is important that you bring the following to your appointment:

  • Your medical and dental cards.
  • Current list of all medications you are taking.

PLEASE NOTE: All patients under 18 years of age must be accompanied by a parent or guardian at the consultation.

If your dentist or physician has taken x-rays, you may request that they forward them along with any correspondence to our office by mail or they can email them to referral@farragutoralsurgery.com. If there is not enough time for them to do so, please pick them up and bring them to our office at the time of your appointment. Additional films can be taken at our facility if necessary.

FINANCIAL INFORMATION

Our Office Accepts The Following Forms of Payment: Cash, Checks, CareCredit, Visa, MasterCard, American Express, Apple Pay and Discover.

Covered by Insurance

For Our Patients Who Have Insurance: Our office will be glad to file your insurance claim(s) for you to companies we are contracted providers with. However, please be aware you are ultimately responsible for the total fee. The office will verify your insurance coverage from the information that is supplied by you. A representative from our office will call you with the estimated amount that will be due on the day of surgery or due one (1) week before your surgery if the surgery is scheduled in the hospital. Please know that there may also be additional payment requested if the insurance does not pay as expected.

If your medical/dental insurance requires a referral to see a specialist, it is the patient’s responsibility to obtain this before being seen in our office.

Self-Pay

For Our Self-Pay Patients: The total fee is due at the time the service(s) are rendered or due one (1) week before your surgery if the surgery is scheduled in the hospital.

Our office accepts most major medical and dental insurances. Please call our office with any questions you may have.
